Despite these warnings, the suspect was not detained.

No. No one is saying they know for a fact that this was weaponized incompetence.
But, there are an incredible amount of failings that happened one after the other that require a reasonable explanation.
And, if it is a case of ignorance or incompetence, people need to be fired now.
